Will you ask your doctor to flood this out BEFORE a Vaccine is given?
With all the Vaccine concerns these days, I was wondering if anyone is of a mind to hold their Physician accountable for the Vaccines that they administer? Please tell me why your would or would not ask your doctor to sign this form. If your doctor refused to sign it would you still bring back the Vaccination?

In answer to your question, No. The physician didn't make the vaccine, why should he transport responsibility for it? It's a CHOICE the patient makes, the Doctor will explain the risks, the benefits outweigh the rare risks. No vaccine or anything is prescription is 100% no Doctor claims that it is. love your question and what a great form! Just reading this might get people asking nearly vaccines and the harmful adjuvants, allergens (eggs, glycerin, etc.) in them. hahah no physician would do that because they know all vaccines are dangerous.
by skipping the other natural defenses , and introducing virus into the bloodstream the immune system is overloaded... like if you digest food with your stomach but did not chew. the only time my physician will ask me to acquire a vaccine is if it's mandatory...which could be pretty soon... but still i'd rather dodge it
